Orthodontics: Child/Teenager Fixed

• This category is for dentists or orthodontists
• The majority of the treatment was carried out using a fixed orthodontic appliance(s)
• The patient was under 18 years of age at the start of treatment
• Minor adjunctive restorative treatment (eg, whitening, recontouring and composite edge bonding) may also have been carried out as part of the treatment plan.

• Detail initial presentation, treatment options and agreed treatment plan (with a discussion as to how this plan was chosen). Describe the treatment journey – include any challenges and how you overcame them. Present the final result and reflect on the case
• Maximum of 1,500 words with up to 26 images
• Please supply as word doc and images as high res .jpg files
• Ensure all relevant consent for publication is attained

Restorative: Single Tooth Composite

• This category is for dentists
• The majority of the treatment was one or two composite restorations with adjacent natural dentition
• Minor orthodontic pre-alignment may have been carried out before the restorations were placed but the major change in the smile will result from the composite restorations.

Restorative: Single Tooth Ceramic

• This category is for dentists with technician
• The majority of the treatment was one or two crowns or veneers with adjacent natural dentition
• Minor orthodontic pre-alignment may have been carried out before the restorations were placed but the major change in the smile will result from the ceramic restorations.

Restorative: Dentures (Full/Partial)

• This category is for dentists with technician
• The majority of the treatment involved a denture or dentures to replace missing teeth. The dentures can be full or partial
• Tooth- or implant-supported overdentures may be entered here
• Partial dentures retained by crowns may also be entered in this category.

Restorative: Full Mouth Rehabilitation

• This category is for dentists with technician
• The majority of the treatment employed implants, crowns or veneers or bridges (or any combination) to treat the majority of teeth in the maxilla and mandible
• Other treatment may have been carried out, but the major change will result from the prosthetics.

Restorative: Composite Smile Makeover

• This category is for dentists and dental therapists
• A case where direct composite resin was applied to multiple teeth to improve the smile. The composite should be directly applied into the mouth, not made via CAD/CAM
• Minor orthodontic pre-alignment or periodontal plastic surgery may have been carried out before the restorations were placed but the major change in the smile will result from the restorations.

Restorative: Ceramic Smile Makeover

• This category is for dentists and dental technicians
• The majority of the treatment employed indirect restorations (crowns or veneers or bridges (or any combination) to treat teeth in a single arch. There was little or no restoration of the opposing arch. Indirect composite smile makeovers should be entered here
• Other treatment may have been carried out, but the major change will result from the prosthetics.

Tooth Whitening

• This category is for dentists or DH&Ts under the prescription of a dentist
• The primary aesthetic treatment was whitening
• Minor orthodontic pre-alignment or minor restorative treatment may have been carried out during the execution of the treatment plan, but the major change in the smile will result from the bleaching.

Implant: Single tooth

• This category is for dentists and/or technicians. If a dentist is entering alone, the technician should be named on the entry form – both the dentist and technician will be awarded. If different clinicians provided the surgical and restorative stages of the treatment, all should be named in the entry
• The treatment involved replacement of one anterior tooth using implants to support the restoration
• This can include immediate/delayed placement and/or immediate/delayed loading
• Other treatment may have been carried out, but the major change will result from the implant therapy.

Implant: Multiple teeth

• This category is for dentists and/or technicians. If a dentist is entering alone, the technician should be named on the entry form – both the dentist and technician will be awarded. If different clinicians provided the surgical and restorative stages of the treatment, all should be named in the entry
• The treatment involved replacement of multiple teeth using implants to support the restoration (this may be a small anterior bridge or two adjacent implants). Excludes full arches
• This can include immediate/delayed placement and/or immediate/delayed loading
• Other treatment may have been carried out, but the major change will result from the implant therapy.

Implant: Complex

• This category is for dentists and/or technicians. If a dentist is entering alone, the technician should be named on the entry form – both the dentist and technician will be awarded. If different clinicians provided the surgical and restorative stages of the treatment, all should be named in the entry
• Where the clinical presentation posed significant clinical challenges to success treatment
• Cases suitable for this required extensive tissue regeneration/grafting, treatment of peri-implantitis including implant resection/removal, BRONJ and osteonecrosis or other catastrophic tissue loss
• This can include immediate/delayed placement and/or immediate/delayed loading
• Other treatment may have been carried out, but the major change will result from the implant therapy.

Facial Aesthetics: Botulinum Toxin

• This category is for any clinician providing facial aesthetic treatments
• The case will involve botulinum toxin treatment in the upper or lower face.

Facial Aesthetics: Dermal Filler Perioral

• This category is for any clinician providing facial aesthetic treatments
• The case will involve treatment of the lips or nasolabial region and marionette region, or a combination of the entire perioral region.

Facial Aesthetics: Full Facial Treatment (incl. thread lift)

• This category is for any clinician providing facial aesthetic treatments
• The case will involve full face treatment, involving a combination of two or more treatments, including a resorbable thread lift plus botulinum toxin, dermal fillers and/or platelet rich plasma (PRP).

Best Endodontic treatment

• This category is for dentists and endodontists
• The case will involve treatment of a single root canal
• Other treatment may have been carried out, but the focus here will be on the standard of the root canal therapy.

Endodontics: Retreatment

• This category is for dentists and endodontists
• The case will involve root canal retreatment, detailing why the previous therapy failed
• Other treatment may have been carried out, but the focus here will be on the standard of the root canal therapy.

Periodontics: Surgical

• This category is for dentists, periodontists and DH&Ts
• The case will involve treatment of periodontal disease using surgical periodontal therapy, including gum and bone re-contouring, pocket depth reduction and guided tissue regeneration
• Other treatment may have been carried out, but the major change will result from the periodontal therapy.

Transformative treatment: not smile makeovers, but rather trauma, or facial disfigurements, or lack of function/appearance profoundly affected by medical conditions. These are life-changing cases. It can inclusive multi-disciplinary work

• This category is for all: dentists, technicians, DH&Ts
• The category isn’t for your standard smile makeover, it’s for the cases that have changed lives
• The case will involve treatment of trauma, facial disfigurements and/or lack of function/appearance profoundly affected by medical conditions
• There’s no stipulation over what treatment was undertaken; the treatment has to have been extensive enough to transform the patient’s life.
